Confidentiality & Privacy Policy
The therapy process at the Limina Institute is grounded in your right to privacy and the right to confidentiality in professional relationships. While it may prove necessary to provide some information to other persons directly involved in providing medical and/or further psychotherapeutic help to you, this information would only be communicated to someone after first requesting your consent.
There are several important exceptions to confidentiality that are legally mandated. In general terms, these exceptions include: 1) I must notify relevant others if I judge my client has an intention to harm another person, 2) I must report child abuse, neglect, or molestation to the Department of Child and Family Services (DCFS) as required by law. I must also report dependet aldult or elder abuse, 3) in legal proceedings, I and/or my records may be subpoenaed by the court, and 4) I may need to notify relevant others if I judge that my client is at risk of hearming themselves.
Confidentiality will be respected in all cases, except those noted above. If you request that I disclose information to another person or professional, I must have your written permission to do so. In such cases, I have release of information forms, which can be signed by you requesting that information be disclosed.
